Once you have made your initial appointment, you can pre-register online by simply clicking on the links below. Please complete the forms. You may download the forms, print them, fill them out and bring them with you to your visit.
Completing the forms prior to your visit will help to speed up the new patient registration process and get you in to see your physical therapist faster!
Our therapists allocate extra time for new patient evaluations so that our therapy plan will be thorough and comprehensive. Because of this, it is our hope that our patients will make every effort to keep their scheduled appointment. It is our policy that a patient should cancel his/her evaluation appointment NO LESS THAN 24 HOURS prior to their scheduled appointment time in order to avoid a cancellation/no-show fee.
